The Central Government Health Scheme (CGHS) provides quality medical care to Central Government employees, pensioners, and their eligible family members through a network of Wellness Centres (Dispensaries), Government Hospitals, and Empanelled Private Hospitals across India.
The CGHS Card Registration process involves enrolling beneficiaries to receive a plastic photo ID card (Smart Card) that enables cashless treatment and access to medical facilities. The card serves as proof of eligibility under the CGHS scheme and must be shown during medical visits or hospital admissions.
Central Government employees (serving)
Central Government pensioners
Eligible family members/dependents
Members of Parliament, Judges, and Freedom Fighters (as per norms)
Certain autonomous body employees as notified by the government
For Serving Employees:
Duly filled CGHS application form (available on cghs.nic.in)
Recent passport-size photographs of self and dependents
Copy of government ID card/office identity card
Proof of residence (Aadhaar, utility bill, etc.)
Proof of age for dependents (birth certificate/school ID)
For Pensioners:

CGHS contribution is made monthly (for serving employees) through salary deduction, or one-time (for pensioners) based on the level of pay or pension.

Serving employees: Through salary deduction.
Pensioners:
Online payment via Bharatkosh Portal (https://bharatkosh.gov.in)
OR through Demand Draft (DD) in favour of “PAO, CGHS” payable at the respective city.
After successful registration and document verification, the CGHS card is issued and can be collected from the respective Additional Director, CGHS Office.
The card contains a unique beneficiary ID and details of all dependents.
