The ECHS Polyclinic Change process enables an Ex-Serviceman (ESM) or their dependents to transfer their registered ECHS Polyclinic from one location to another. This facility is provided by the Ex-Servicemen Contributory Health Scheme (ECHS) under the Ministry of Defence, Government of India, to ensure that beneficiaries continue to receive uninterrupted medical care even after relocation or change of residence.


Purpose of Polyclinic Change:

The main objective of the ECHS Polyclinic Change is to:

  • Provide continuous medical facilities to ESMs and their families after relocation.

  • Update the ECHS database with the member’s new residential address and nearest Polyclinic.

  • Ensure that medical records, referrals, and prescriptions are transferred seamlessly.

  • Facilitate better access to Empanelled Hospitals and Labs near the new location.


Eligibility for Polyclinic Change:

The following categories are eligible to request a change:

  1. Ex-Servicemen (ESM) who have shifted to a new city, district, or state.

  2. Widows or Dependents of ESMs who have changed their residence.

  3. Retired Defence Personnel posted in a new area under the same or different Regional Centre.


Documents Required for ECHS Polyclinic Change:

Basic Documents:

  • ECHS Smart Card (Primary Member)

  • Discharge Book / Service Book Copy

  • PPO (Pension Payment Order)

  • Aadhaar Card (Primary & Dependents)

  • Proof of New Address (Aadhaar / Electricity Bill / Rent Agreement / Property Papers)

  • Mobile Number & Email ID

Additional (if applicable):

  • Application for Polyclinic Change (typed or handwritten in prescribed format)

  • No Objection Certificate (if required by Station HQ)

  • Dependent Proof (for family members included in the card)

  • Old Polyclinic Card Copy (if available)


Step-by-Step Process for ECHS Polyclinic Change:

Step 1: Application Submission
Submit a written or online application for “Polyclinic Change” to the respective Station Headquarters or Regional Centre ECHS.

Step 2: Attach Documents
Attach copies of ECHS Smart Card, Discharge Book, PPO, and proof of new address.

Step 3: Verification
Station HQ verifies service details and new address authenticity.

Step 4: Approval by Regional Centre
Regional Centre ECHS reviews the application and approves the transfer.

Step 5: Update in ECHS Portal
Once approved, your ECHS Smart Card and online records will reflect the new Polyclinic name and location.

Step 6: Use New Polyclinic
After approval, you can avail medical services, consultations, and referrals from your newly assigned ECHS Polyclinic.
